Health Questionnaire

Ensuring you are safe and ready to exercise is our upmost priority. Before all sessions if you could please ensure this has been filled out.

I have read, understood and accurately completed this questionnaire. I can confirm I am voluntarily engaging in an acceptable level of exercise, and my participation involves a risk of injury.

If I answered Yes to one of the questions above, I have sought medical advice and my GP has agreed that I may exercise.

I understand in The Fitness Centre there are live CCTV cameras and this will be stored securely and following GDPR rules. 

I give permission for JDD Fitness to post photos and videos on their Website and Social Media Platforms.

JDD Fitness is not a medical doctor or a registered dietician and agree to the following that they are providing recommendations, not prescribing advice that should disregard that of any medical physician of whom is responsible for your health and wellbeing, and that I follow the advice provided in this document at my own risk.

I release the PT from any liability for personal injury or other damage I may sustain whilst engaging in any exercise programme suggested by the Personal Trainer.

If any of the above information should change, I will let my instructor know at the earliest opportunity

Group Session

-Direct debits will be taken through the payment provider stripe.

-There is no contract, you can upgrade or downgrade your plan, or cancel the plan at any time. The notice period to do any of these is 2 weeks,  a part payment may be taken or there may be a period of time before the payment amount is changed.

-You are able to use the session credits at any point during the billing month, you can cancel a session at any point with no cancellation payment required.

(All i ask is for you to cancel off the app or message me prior, failure to do this will count as using your credits, as someone else could use this slot.

-The app will automatically give you the amount of credits, these will reset each month, failure to use all of them will cause them to be lost.

-If you are on holiday for 7 days for example, I will work with each case to transfer 1/4 of your credits to the next month. This means if you don’t use them that calander month they are not lost, atleast 10 days notice is required for this to happen.

-On a 5 week calander month, you may not have enough credits to do all of the sessions you want, at any point I can add extra credits for you, (£5 individual or £8 double).

-Session cancellation- if I am away for 7 days, I will provide an online workout for each session alongside 1/4 of your monthly credits that can be used  at any point in the 3 months following this so you do not loose these.

Depending on the number of sessions I miss will depend on the number of credits you recieve which is dependent on your chosen package.

-I have the right to cancel / amend sessions at any point. By accepting to the direct debit you can confirm that you are happy with the above. Any questions on any of these please let me know.

12 Week Program / Evolve Coaching

-Direct debits will be taken through the payment provider stripe and 3x Payments will be taken

-£20 Deposit will secure you place on the program and this will be taken from the 1st Monthly Payment

-For Face To Face messages you are able to use the session credits at any point during the billing month, you can cancel a session at any point with no cancellation punishment.(All i ask is for you to cancel off the app 2 hours prior, as someone else could use this slot)

-The program is for a 3 month period and the cost is spread over the 3 months so there is a 3 month commitment for payment of the program

By proceeding with the program you accept - 'Joshua Drake' of JDD Fitness is not a medical doctor or a registered dietician and agree to the following

: (1) 'Joshua Drake' is only providing recommendations, not prescribing advice that should disregard that of any medical physician of whom is responsible for your health and wellbeing, and that I follow the advice provided in this document at my own risk.

(2) You have provided 'Joshua Drake' with all the necessary information regarding any important details with regards to your medical history and/or any current medical treatment(s)/implication(s) which would impede your ability to follow the advice given in this document.

(3) 'Joshua Drake' does not guarantee specific results, accept that the advice outlined in this document is tailored to guide me towards the desired outcomes agreed between 'Joshua Drake' and myself, that it is my responsibility to follow the advice given in this document to the best of my ability and that 'Joshua Drake' will not offer refunds for any perceived lack of progress.

(4) That if there are any changes to your health and wellbeing or if you are concerned that/believe that any advice contained within this document has compromised the former forthwith, that you will communicate with 'Joshua Drake' at the earliest convenience and that you accept all responsibility for any consequences if you fail to do so.

Personal Training

-Direct debits will be taken through the Payment Provider Stripe. -There is no contract, the notice period for cancellation is 2 weeks, a part payment may be required for the notice period. To cancel a booked session, the Client must notify the PT either in person or by phone, text or email 24 hours prior to the appointment, If less than 24 hours’ notice, the full session fee will be taken. If more than 24 hours notice is provided, this can be transferred to another date. This session must be used in the next 2 calendar month, if the banked session isn't used it will be lost - If the PT cancels a session with less than 24 Hours prior to the session, you will recieve a Free Session / be credited the money back. -The PT is only required to wait for 15 minutes past the agreed start time. After this the PT may leave the premises and the client will forfeit the amount for the session. If the Client, arrives within the 15 minutes the PT will complete the time left of the original session. No extra time will be given. -Payment is taken for 48 weeks of the year therefore a 4 week month every month. There will be months with 5 weeks in, however payment is still only taken for 48 weeks of the year giving 4 weeks to account for holidays and occasions. I will note all of my holidays from the date of the direct debit and for any reason this is over 4 weeks you will be refunded the missed sessions. -I have the right to cancel / amend sessions at any point. By accepting to the direct debit you can confirm that you are happy with the above. Any questions on any of these please let me know.
